About Registration: Please note the specific program you are wishing to register for and carefully select the appropriate paperwork. Download correct PDF, Print form and completely fill out. Mail in forms with check (deposit/payment).

Paperwork may include: 1) Registration, 2) Waiver Release, & Assumption of Risk, 3) Health Form/Statement (w/ Current Medications Form).

Going out with Calleva and need a waiver, make sure you have filled out our Standard Waiver before you participate in a program!

2010 Summer Registration:

Registration is now open to all campers -new and old!

  • Anyone can Register through snailmail with downloaded pdf below (mail w/ deposit payment).

Calleva's Online Registration process allows families to complete the registration forms and pay all camp fees in full via credit card through our secure server. If you do not wish to pay camp fees in full at this time, please download the PDF version of registration below and send it to us by mail with a $150/week deposit. Camps fill on a first-received, first-registered basis. Here's an overview of the 5-step on-line process:

  • Click on the link and enter camper(s) information
  • Choose camps from the schedule by clicking on the "+" sign
  • Choose extra services and transportation route
  • Complete Parental Contact information form
  • Proceed to checkout
  • Pay total fee via credit card
  • Print your confirmation page for your records and download Heath and Waiver forms

    • Online registrations will still require families to download and complete Health Forms and Waivers for each camper. These forms will be due to Calleva offices by May 1, 2010. Submitting online registration implies your acceptance of Calleva's withdrawls/dismissals, agreement, waiver and payment policies.
